TEAM Registration Form
Team Info must be submitted with $200 Deposit no later than 10 days prior to League Start Date. This is the required Application Form for all NEW teams, all sports.
Player Registration/Release Form (REQUIRED)
Required by ALL Soccerville Participants.
Team Roster Form (REQUIRED)
Each Team must submit BEFORE first game
Players may be added/dropped up to Game 2.
Each Player on a Team Roster must also have a CURRENT Player Registration Form/Waiver on-file BEFORE participation.
Players can only be on ONE Team Roster in any League/Age.
